How to request a transcript

You may submit a transcript request in one of three ways:

  1. Come in to the Kentucky Wesleyan College's Registrar’s Office, located on the first floor of the Bernard-Jones Administration Building, to complete and submit a short request form,

    OR

  2. Use our Transcript Request Form which may be filled in online, printed and mailed with your payment to:
    Registrar
    Kentucky Wesleyan College
    3000 Frederica Street
    Owensboro, KY 42301

    OR

  3. Send a written request to the address above. Please include the following in your written request:
    • Your full name (including maiden names)
    • Your social security number
    • Your current telephone number
    • The address(es) to which you want the transcript(s) sent
    • A written signature
    • A $5.00 check or money order per copy requested made payable to Kentucky Wesleyan College

We try to process each request within five days of receiving this information.

Please be aware that we are not responsible for transcripts or transcript requests that are lost in the mail.

Unfortunately, we are not able to fax official transcripts for reasons of security and privacy. For these same reasons, we are unable to take transcript requests via email.

If your request is incomplete or is not able to be processed for any reason, we will attempt to notify you by telephone. After that, we will file your request for three months before destroying it.
